The East Bay’s Best Doctors

Is Yours on the List?

     Your grandmother has just been diagnosed with Alzheimer’s, and you want to ensure she’s seen by a top-notch neurologist. Or maybe you’ve been trying unsuccessfully to get pregnant and want to consult with a fertility specialist. Rather than turning to friends and family members for names of trusted physicians, what if you could ask your doctors to whom they would go for medical assistance?
Thanks to the organization Best Doctors Inc. (bestdoctors.com), the public can now obtain specific recommendations from top physicians. The organization, founded in 1989 by Harvard Medical School–affiliated physicians, says its goal is to give consumers information about specific diagnoses, the latest medical advances and where to receive cutting-edge care. Every two years, Best Doctors polls physicians asking, “If you or a loved one needed a doctor in your specialty, to whom would you refer them?” Only physicians identified by their peers in previous years as “best doctors” receive the survey.
     Out of the 220 physicians named Best Doctors in the East Bay, we chose five outstanding physicians to profile. Each one is a recognized leader in his field. Spanning a wide variety of specialties from neurology to gastroenterology to reproductive medicine, this list of local physicians serves as a guide for patients seeking some of the best medical specialists the area has to offer.

Dr. Eric Lipsitt

Internal Medicine
Kaiser Permanente Oakland Medical Center
     A native of Vermont, Lipsitt was an English major at Yale University before he decided to follow in his father’s footsteps and pursue a career in medicine. After attending medical school at Case Western Reserve University, he served his residency at Kaiser Permanente Medical Center Oakland where he still continues to practice internal medicine. The father of a grown son, Lipsitt, 53, lives with his wife
in Oakland.

You began working at Kaiser Permanente’s Oakland Medical Center as a resident more than 20 years ago and still practice internal medicine there. What has kept you inspired through the years?
     It’s rewarding to care for several generations of the same family. I have grandparents and parents who have been my patients for years, and I’m now beginning to care for their young adult children. I even have one patient that I started seeing when I was a resident. I think I’ve built a lot of good relationships with my patients over the years. I also like how Kaiser works as an integrated delivery system with our doctors, our hospitals and our health plan. As doctors we are independent in our medical decision-making. I don’t have to receive pre-authorization from an insurance company for any medical services that I provide to my patients, and I can refer them to a specialist, most of whom I know, without worrying if their visit will be covered by the health plan.

How has the field of internal medicine changed  over the past two decades?
     When I graduated from medical school, AIDS was just starting to become a worldwide epidemic, and it was a death sentence for many patients. Today, we have many excellent medications available and can treat AIDS as a controllable chronic illness, much like diabetes. Laparoscopic, or minimally invasive, surgery has also changed the way we treat many medical conditions. Years ago, someone with
a hernia was likely to be hospitalized for a week after surgery. Today, unless there is a cause for concern, hernia repair can be done on an outpatient basis.

What are some of the advances in medical technology that have helped you in your practice?
     Early on in my career, we used to have to wait weeks for the results of CT scans and x-rays. Now the entire process is computerized, and I can typically get a patient’s test results the same day. Kaiser Permanente members now have the ability to e-mail their doctors and obtain their test results online. When one of my patients goes in for blood work in the morning, we will both receive the results of their lab work via e-mail later that day.

How have your patient demographics changed over the years?
     I believe that we have gotten a lot better at diagnosing and controlling high blood pressure and cholesterol. A recent study conducted by our Department of Care and Service Quality found that Kaiser Permanente members in Northern California are 30 percent less likely to die from heart disease than nonmembers. As more of our population becomes overweight, we are seeing more obesity-related diseases, including diabetes. I work with my patients to eat better, exercise regularly and manage their stress.

Dr. Robyn Young

Neurology,
Private Practice, Alameda
     Young was born in Danville, Ill., and moved to California in 1969 to pursue her undergraduate degree in chemistry at Stanford University. After graduation she went on to Yale Medical School with plans to go into research. After completing an internship and then a residency in internal medicine at California Pacific Medical Center and a neurology residency at the University of California, San Francisco, Young decided she would rather work with patients than behind the scenes doing research. Those who visit her thriving practice on Marina Village Parkway in Alameda are glad Young made the decision to pursue a career in clinical work. An active member of the Rotary Club of Alameda, Young, 58, resides in Alameda.

What has been your biggest challenge as a neurologist?
     Over the last 25 years, I have watched the insurance industry take over healthcare, subverting and manipulating medicine in order to deliver greater profits and higher executive compensation, while sacrificing the health of those who have been paying escalating premiums for years. People believed they had great coverage only to find when they needed it, their insurance company found a way to drop them or inadequately cover what they needed, despite hours of appeals. I have been a longtime advocate of healthcare reform and a proponent of single-payer healthcare in California (SB810), though I believe that nationally a public option is the most reasonable compromise between those who choose to purchase private insurance and those who choose to purchase a plan backed by the government, similar to Medicare.

Has there been a lot of progress in how neurological conditions such as Alzheimer’s and multiple sclerosis are treated?
     We have better treatments now than we did years ago, and in the near future, we may have neural protective treatments that can slow the progression of many neurological disorders and keep people in a milder stage for longer. We are now able to treat migraines with medication,
and if stroke victims receive immediate care, they are often able to make a full recovery and to reduce their risk of future strokes. We have medications now that better manage the symptoms of multiple sclerosis and slow the progression of the disease. We also have medications that can improve the function of patients with Parkinson’s disease. While these aren’t cures, they can help patients have a better quality of life than they did years ago.

Have there been any new technological advances in the neurology field in recent years?
     There have been many new technological advances, and there are more on the horizon. In epilepsy there is possible surgical resection, vagal nerve stimulation and, in the future, possible other forms of electrical or magnetic stimulation in addition to new medications. For some people with Parkinson’s, or with dystonia (a type of involuntary twisting of one or several limbs), DBS (deep brain stimulation) may be strikingly helpful. Using stem cells to replace missing neurotransmitters in Parkinson’s and other disorders is probably not far off. Microvascular and robotic surgical procedures are also increasing, allowing doctors to perform microscopic operations on parts of a patient’s brain that might have been considered risky or impossible before.
 
How has your practice changed over the years?
     I have developed an interest in neurologic complications of autoimmune diseases, so I see a larger number of people with multiple sclerosis or with neurological complications of systemic lupus erythematosus (SLE), rheumatoid arthritis or other related disorders.

Dr. Janie Hirata

Obstetrician and Gynecologist,

     As the mother of two children, Hirata, 53, says one of her greatest joys is helping patients realize their dreams of becoming parents. Hirata attended medical school at Wayne State University in Detroit, Mich., and completed her residency and fellowship there before joining the staff of Kaiser Oakland in 1992. In addition to her clinical work, Hirata enjoys teaching resident physicians as part of Kaiser’s Residency Research Program. She lives with her family in Piedmont.

As a reproductive endocrinologist or fertility specialist, you treat patients who are experiencing infertility. Can you give an overview of what this entails?
     Infertility affects 15 percent of reproductive-aged couples. Our reproductive endocrinology clinic at Kaiser Oakland is one of six active clinics in Northern California. The Kaiser Permanente reproductive endocrinologists all work together as a team, and we consult with each other frequently. We typically start patients with a basic infertility examination that evaluates fertility factors including ovulation, egg quality, fallopian tubes and sperm. We also check patients for endocrine problems such as a low thyroid or undiagnosed diabetes that can lead to fertility problems. If a patient is having problems with ovulation, we will recommend an oral ovulation medication or appropriate lifestyle changes. Hormone shots and insemination are also viable options for many couples. If we have exhausted all of our other options, we will refer couples to Kaiser’s in vitro fertilization clinic in Fremont and discuss other options such as adoption.

How have your patient demographics changed over the years?
     We’re seeing patients who are older and have delayed having children. Getting pregnant after the age of 35 can be problematic, and some of these women have chronic health problems such as diabetes and hypertension and various uterine disorders including fibroids and endometriosis, all of which can make it even harder to conceive. On the other hand, patients are also more health-conscious and open to exploring different health options including alternative medicine procedures that may help them to conceive. We see a very diverse group of patients including traditional and nontraditional families.

What kinds of advances have you seen in reproductive health?
     Advances in reproductive technology have given hope to women who are cancer survivors and who hope to conceive. Women battling cancer can now freeze their eggs before undergoing cancer treatments. While freezing sperm has been done for decades, egg freezing, or oocyte cryopreservation, is a relatively new option available to women who hope to achieve pregnancy after undergoing cancer treatments. IVF has also improved significantly, and we now have great success in transferring one embryo rather than two or more which also minimizes the risk of multiple births.

What do you like best about your job?
     I work with a great team of professionals, and I believe that we bring hope to many people who might not have hope otherwise. It’s not uncommon for us to see the same patient over the course of a year or even to see a patient and her partner return to us when they are trying to conceive again, so we are able to form real relationships with our patients and their families. It’s an exciting field!

Dr. Olajire Idowu

Pediatric Surgeon, Children’s Hospital & Research Center Oakland

     Born and raised in Nigeria, Idowu has been practicing medicine in the United States for more than 30 years. He graduated from the Government College in Ibadan, Nigeria, and studied at the University of Ibadan School of Medicine. Idowu completed his internship at the Baptist Medical Centre in Ogbomoso, Nigeria. He joined the Children’s Hospital staff in 1991 and was named chairman of the surgery department two years later. In addition to his work performing a variety of pediatric surgeries at Children’s, Idowu founded Medical C.A.R.E. Inc. (medicalcareforchildren.org) with three of his colleagues in an attempt to teach surgical and other lifesaving procedures to medical teams in Nigeria and Belize. Idowu, 63, is married and has two grown children. He lives in Lafayette.

How has the field of pediatric surgery evolved over the last several decades?
     There have been a lot of advances in minimally invasive surgery techniques, and we are able to do more laparoscopic procedures involving the abdomen and thoracoscopic procedures involving the chest. By performing these procedures with small incisions, our patients experience a number of advantages, including faster healing and shorter hospital stays. Pediatric anesthesia has also improved over the
years and is much safer, provides better outcomes and few adverse side effects in children.

What do you hope to accomplish with your nonprofit humanitarian medical group, Medical C.A.R.E. Inc.?
     After going on a medical mission to Nigeria in 2000, I approached several of my colleagues at Children’s with the idea of bringing medical care to children in Third World countries. To date, our team of physicians, nurses and respiratory therapists has been to Nigeria and Belize and have faced strong limitations such as lack of electricity and water. We have trained their local pediatric medical professionals on how to care for a variety of conditions through workshops and bedside training and have also taught them how to use some of the medical equipment that we’ve donated. The goal of our organization is reflected in our name: “Care for Children of All Races Everywhere.”

How have your patient demographics changed over the years?
     At Children’s Hospital, we see children ages 0 to 18 with a variety of conditions ranging from appendicitis to heart deformities every day. In recent years we’ve seen an increase in the number of families who can’t afford care because they don’t have insurance. Our foundation helps pay to ensure care for families who couldn’t otherwise afford to pay.

As a pediatric surgeon, what’s your biggest challenge?
     As a doctor and a father, I love helping children, but it’s hard to realize there are times when even doctors can’t cure all of
a child’s health problems.

Dr. Subhendu “Bob” Narayan

Gastroenterology

     A native of India, Narayan, 49, knew at a very young age that he wanted to become a doctor. After receiving his undergraduate degree in microbiology from Rutgers University, he went on to attend medical school at Cornell University. Relocating to California after graduation, Narayan completed his internal medicine internship and residency at the University of California, San Francisco Medical Center. After deciding to specialize in the field of gastroenterology, Narayan completed a gastrointestinal fellowship at the University of California, Los Angeles and went on to complete an advanced endoscopy fellowship at the Cleveland Clinic in Ohio. Today, Narayan practices at Alta Bates, Alameda and Highland hospitals and lives with his wife and two children in Walnut Creek.

When did you first decide to pursue a career in medicine?
    As far back as I can remember, I’ve had an interest in science and biology. My uncle was a physician, and he used to take me on rounds with him when I was younger. I liked how the field of medicine combines technical aspects with caring for patients.

If you could offer patients one piece of health advice, what would it be?
     Focus on prevention by trying to maintain a healthy lifestyle. We all know that eating right and being active are good for us, but adopting these habits can also cut your risk of disease. Obesity raises your risk of a number of conditions including gastroesophageal reflux disease, fatty liver disease, colon cancer and gallstones.

How has the field of gastroenterology changed over the years?
     The number of colonoscopies that we do has dramatically increased. More people are choosing to get a colonoscopy these days as patients realize that lives can be saved with early screenings for pre-malignant polyps and cancers. Our guidelines recommend that beginning at age 50 all patients get a colonoscopy, unless there is a family history, making earlier screening important. Thanks to the increase in colonoscopies, we’ve seen colon cancer rates declining for the first time in years. We’ve also seen a lot of advances with Hepatitis B, a condition that is common in the Bay Area among many people of Southeast Asian origin. Fifteen years ago there was no reasonable treatment for the disease; now there are multiple options.

What is the biggest challenge you currently face in your practice?
     With the limited time that doctors have these days, I think it’s still imperative to get to know your patients and to pay attention to aspects of their lifestyle that may have an impact on their health. As doctors, I think we can learn so much from our patients.
